Consent to Specimen Collection and/or Rapid Testing

I hereby consent to specimen collection and/or rapid diagnostic testing performed by a licensed Medical Laboratory Scientist from Lab On-The-Go (LOTG).

1. Qualification of Operator. All services are performed by a licensed MLS trained to ISO 15189 quality standards.

2. Nature of Testing. Rapid test results are screening results and may require confirmatory laboratory testing.

3. Confidentiality. My personal information and test results are strictly confidential and will not be shared with any third party without my explicit written consent, except where required by Jamaican law.

4. Right to Refuse. I have the right to refuse or discontinue testing at any time before or during the visit without penalty.

5. Results Delivery. Results will be delivered directly to me via the WhatsApp number provided.

6. Accuracy of Information. I confirm that the medical information provided in this form is accurate to the best of my knowledge.

By submitting you confirm all information is accurate. We will confirm your appointment via WhatsApp within 2 hours (Mon–Sat 7am–8pm). Payment is due on the day of your visit — cash or bank transfer accepted.
